student post /Virginia Retti
Overall, my informatics competency levels are strong in knowledge, but weak in actual skills.  Besides using the Epic system in my hospital to access patient records and policies and procedures, my ability to search for information on a larger scale using the software provided by my institution is limited.  This is largely in part due to not needing to use it, as my current role does not require it.  If, for some reason, I needed to utilize it to improve patient care, I would defer to my co-workers or the IT department to guide me to the appropriate location.

One strength that I have related to informatics competency is utilizing the Epic program to retrieve data needed to provide patient care and implement appropriate actions based on that information. I am able to see the patientâ€s entire medical history and new information entered during the hospital stay. I can also enter my own data into the program, which allows other team members to track the patientâ€s care throughout their hospitalization, as well. By accessing all the care teamâ€s input, patient care is improved and carried out more effectively.

Another strength is that I am able to use multiple search engines to obtain the information and knowledge needed to perform my job more effectively. It is extremely important to stay up to date on new evidence-based information to implement changes to practices and procedures based on those findings.
A competency that I need to work on is the ability to correct some of the defects observed in working with systems. Correcting a defect in a system is difficult because data changes all the time, so a correction that needs to be addressed today may be completely different tomorrow. “The concepts we use in biomedicine often evolve over time. For example, the above-mentioned definition of MI, which was published in 2018, is the fourthuniversal definition of MI that builds on prior nonuniversal definitions beginning in 1971. Conceptual drift makes information needs analysis harder, and also interacts with information imperfection, because information that is correct at one time, may not be correct later once a concept is refined.” (Johnson & Bernstam, 2023)

Another competency that I lack skills in is participating in the selection, design, implementation and evaluation of systems processes. Johnson & Bernstam (2023) note that one of the major challenges at the information level is formalization. To design an information system to address a biomedical information problem, we must determine what information is needed to solve a specific problem and what information operations need to be performed on that information. Information needs analysis requires a precise definition of the problem we are trying to solve; however, due to the complexity and dynamic changes in biomedicine, even the best problem definitions are oftenapproximationsLinks to an external site.. Likewise, it is often difficult to fully anticipate the information needed to solve a problem.
One strategy that I need to enhance my deficient informatics competencies would be recognizing a defect within the system and forwarding the information to the appropriate department. Nurses in my department are not responsible for personally correcting errors that we discover; the errors are usually left unreported and remain unchanged until someone eventually reports them.
Identifying the appropriate personnel to address any changes that I discover is the first step to involve myself in the process of obtaining implementation of a necessary change and/or improving the existing systems process.
Rather than rely on a co-worker to note the defect when it is found, I will forward my findings and follow up as needed.
The importance of accurate data reporting cannot be said enough. Checking and re-checking data input and information, if not done properly, can hinder patient care.  “Ensuring data accuracy is of paramount importance because even a minute discrepancy due to data entry or measurement inaccuracies can negate a moderate correlation or invalidate a statistically significant finding [1, 2]. When data are manually recorded, either by being typed into a medical record or captured through measurements, the risk of introducing mistakes increases.” (Kuwaye & Cho, 2023)

Professor question Dr Prasad
Jillian,
Thank you for your post this week. Clinical Information Management are critical skills for APNs. These competencies include:
Clinical Information Management

Ability to use clinical decision-making systems (CDS) and system safeguards to protect patients and protected health information (PHI)
Ability to request and evaluate reports for informed decision making
Expertise to participate in the selection, design, and evaluation of clinical information systems and patient-care technologies
Ability to use available technologies to communicate appropriately and effectively

Question for you to reflect on: Which competencies will you need to work on related to clinical information management in the APN role?
