The task Taking a decision you have been involved in with regard to the care and/or treatment of a client/patient, critically analyse that decision making process paying particular attention to team working, leadership, legal, ethical and professional issues. You may choose which of the last three concepts you relate to the scenario but must follow and use the headings given below. Essay headings (you must use these) 1/ Introduction Generally and briefly set the scene for the essay. Include a brief referenced statement of confidentiality as this must be maintained throughout the assignment. 2/ Scenario Describe to the reader the key points of the scenario to be analysed. This section should not be overly long, but it must clarify key parts of the scenario in preparation for the analysis that follows. 3/ The decision making process Using appropriate decision making theory critically analyse the decision making process as it occurred within the scenario. This section must be supported with appropriate theory and literature. 4/ Team working and leadership Critically analyse the management of the scenario, using appropriate team working and leadership theory and literature. 5/ Legal, ethical and professional issues You can choose which of these three concepts to link to the scenario and do not have to use all three. Explain this in your introduction. Once you have decided which of the three concepts are relevant, critically analyse the scenario using appropriate theory, literature and professional guidelines, where relevant. 6/ Evaluation Draw your analysis together and give a balanced view on the strengths and areas for improvement of the decision making process analysed above. Also, draw out the most important elements of care improvement in relation to team working and leadership, and finish by identifying, in the final paragraph, key learning for you from this process. My scenario is about an elderly lady, in her late 70s, I would prefer her to be called Mrs Lily, she was initially admited due to a fall and chest infection. Before Mrs Lily was addmited to hospital she had a package of care, she had carers going in to help her in the morning and in the evening. She was treated for the infection and was medically fit for discharge. She had been receiving physiotherapy and was discharged by physiotherapist. As Mrs Lily was medically fit for discharge she was refered to comunity and occupational therapy assessments. Before the nurse could discharge her, all healthcare professionals within the multi disciplinary team had to make sure the patient was safe to go home. The nurse contacted the patient’s daughter with the consent of a patient as she was her next of kin, to inform her of a discharhege which was the next day. However daughter was not happy about discharge as she wanted her mother to go into a nursing home as she felt that she was not safe to be at home by herself. MDT meeting was organised for her to come and join in, the decision was made for Mrs Lily to get discharged to a residential home but still daughter was not happy as she wanted her to go into a nursing home. Mrs Lily ended up getting discherged to a rehabilitation ward while waiting for another package of care.
